As part of our responsibilities, we must produce an annual report to provide a summary of the number of times we have triggered duty of candour within our service.

How have you made sure that the staff team understand your responsibilities relating to the duty of candour and have systems in place to respond effectively?	All staff are emailed our Duty of Candour Policy and asked to read it, staff are prompted to seek advice from Managers if they are at all unsure. An online Duty of Candour course has to be completed through Azilo training for each staff member.
